WAYS TO PARTNER AND COLLABORATE WITH THE Y
ADVOCACY
Join the Y in advocating for policy change with local representatives and senators. Advocate by bringing the Y to the table when a group of community leaders are convening to address community health.
BOARD SERVICE
Experts in the Community Health field volunteer their time and provide their expertise and social connections to move forward the Y’s mission to reduce or delay the social determinants of health through their service as a member of the West Cook YMCA Board of Directors.
FUNDING
To ensure the Y is improving health equity, we must address all the social determinants of health. This leads to a significant cost to underwrite each program including staffing, infrastructure, space, and program implementation.
SPACE
We want to meet community members where they are and increasing access to lifechanging programs. We seek partners to host programs within the communities we serve and/or support our virtual and hybrid environment that includes hosting and maintaining our Virtual Health and Virtual Y platforms.
REFERRALS
Health care professionals and community based organizations can refer patients to the Y through an online system or referrals through the patient’s electronic health record system. Together, we will partner wtih a patient’s healthcare provider to prevent chronic disease.
